We need a survey question option like attached

I'm trying to replace a survey we pay Survicate for, and this is one of the questions we have on our survey.

They make it really simple, you pick either 3 or 5 as the number of answers users can choose from (my screenshot below is the 5 answers option) and the label as well as other settings and thats it, all automatically done.
