Add Rating Scale Matrix Survey Element

It would be brilliant to have a rating scale matrix survey element. This uses a grid to ask multiple related sub-questions using the same set of rating options, such as a Likert scale.
Example -
Survey Item(Rows): Staff Rating, Product Rating, Pricing, Overall Experience, etc.
Scale Rating (Columns): 1, 2, 3, 4, 5, N/A

Also, need true matrix where you define the column headers and row title.

would prefer options to define the column headers and row headers as well so not limited to just ratings. Just overall matrix questions.
