Please add this to make it robust:
- Allow options per gateway (including nmi and others)
- Allow options per payment type (c cards, ACH, Affirm, etc)
- Allow options depending on minimum order value or for all.
- Allow us to edit "fee name" and "fee description". Description should show on a tooltip.

This would be a game-changer! It would be great if a dual-pricing feature could be on a checkout form where customers can choose to pay via check/ACH or Credit Card (where fees can be aded, therefore incentivizing ACH payments).
I understand USA businesses will only be able to accept ACH from USA clients. I'm not sure what the solution is here as to not confuse international customers.

It would be great to be able to calculate fees on receipt value, similar to the way you can provide a discount. For example credit card or processing fees, which are calculated on the total receipt value, not as a 'tax' which is included in product price.

Unless things have changed, it is in violation of Visa and Mastercard's acquiring agreement. You CANNOT charge for credit card processing, it must be included in your price. The legal solution is a cash payment discount, not a processing fee to customers...

Daniel Cowart Here in New Zealand, it's not a violation and it's also not illegal. It's common to pay a surcharge if you're using tap-to-pay and additional surcharge for credit card payments - theres also no law or guidelines around what percentage that surcharge should be

Heidi Wruck same in Australia. It's very common. Most businesses pass the charges on. The legal framework is that we can pass on reasonable costs, and could be required to demonstrate the costs charged to customers are similar to what the business incurs.
