A visual and UX refresh would make a big difference and help bring it up to the standard of tools like ConvertBox. Specifically:

A modernized design system (spacing, typography, component sizing)

Better visual hierarchy and readability

Progress indicators or progression bars where relevant

A cleaner, more modern overall presentation

More flexibility and personalization across form elements

For example, the terms and conditions field is just a plain text box. There’s no way to properly adjust padding, spacing, or styling. More control over layout and styling would significantly improve the user experience.
