Add the ability for form creators to add a repeating section to forms. For example, a doctor's office asks patients to authorize a records request for other offices. The patient would enter the physician's office contact info-- credentials, phone, fax, etc.

There should be an option for an "add another" button that repeats the fields in that table for another row.
