in a gym scenario, when signing up for membership, if a child uses the same phone/email of the parent (who is already a member) when filling a form, the parents info is over-written by the child's data.
there should be a warning before updating existing data when filling a form/survey.

It would be great if first name can also be another criteria for checking duplicates. We don't want the current contact info to be overwritten by a child or a spouse info because they use the same email and or phone number.
