Attached as a pdf to the opportunity and also resides in the documents folder for that specific client. It needs to be secure so anyone with the link can not see it.

Hi it would be very useful from a compliance audit point of view, if a workflow action could also attach the pdf to the contact record. Including the fields completed, meta data, date time, IP etc in essence all the backend data High Level captures from the form. Why? Because if someone says I did not authorise email, text communications etc this data is required to show you are actually compliant.
Yes currently you could trawl through years and years of form submissions to get the data but itz more streamlined and audit ready to have it in the contact record.

Hey everyone - Ashwin here (PM for Workflows).
We’ve been digging into this request and the different ways you’re trying to use a submission PDF today. A big part of the ask here seems to be: “When someone submits a form/survey, I want a PDF copy of that submission emailed out (to the submitter and/or to my team) for an immediate receipt/record.”
A quick update on this - the Forms team is working on an enhancement in the Form/Survey Notification settings to optionally send the submission as a PDF attachment via email notifications which should be coming out soon.
This would cover:
- Auto Responder → email PDF to the submitter
- Email Notification → email PDF to one or multiple configured addresses
Because of that, it looks like we’ll be covering a big chunk of the “email me / my team the submission PDF” need behind this post.
What I’d love help with here is: what workflow-specific use cases remain after that email-notification PDF is available?
If you can, reply with:
- Where do you want to use/store the PDF inside Workflows? What workflow step do you need the PDF to power?
- Do you need the PDF as a true attachment or would a link be fine? (Eg. for channels like SMS, you won't always be able to attach a pdf)
- Do you have any specific workflow logic in mind around this?
Your inputs will help us understand and prioritize the remaining gaps here

Ashwin Raghunandan I want my submission PDF to be added to the documents section of the contact record with the ability to set the name of the file with custom values for organizational purposes.
My clients need the record and a lot of them like to print them out for their review meetings.

Ashwin Raghunandan Hello Ashwin, in our workflow we needed the pdf copy of the submitted forms so that we can save to employee files. For example their job application which was submitted from the website, we created out own logic to build an approval workflow, once the job application is approved, it must be stored in employees profile for 7 years. So if we get the pdf we can store it easily but we also need an approval workflow so that we only store the approved application. Hope this helps.

Ashwin Raghunandan Thank you! I would like the PDF saved in documents automatically, and a link to it would be great as well. I don't have a massive need for the PDFs to be used in workflows personally. Still, I could see where it would be beneficial to be able to add them to an existing or new document so a customer can use them for sms or emails in the future, and to be able to trigger this in a workflow. For example, say we want their initial intake form merged into a new document with a contract, so that now it's all one document, that would be a cool function.

Specifically for salons/med spas/etc it would be awesome to be able to save the PDFs for client consent forms and medical history forms to the client profile (documents section of the contact). This would provide quick and easy access to staff for checking if consent was submitted as well as being able to download the pdf and send in a quick email when clients request a copy.

- I want the submission added to the documents section of the contact.
- The option to CHOOSE (maybe within the automation step?) whether the PDF is sent as an attachment or a link would be ideal.
Additionally, choosing who that PDF (or link) is sent to might be good as well. (Sending it to the contact owner, or specific team member, or even contact ID'd in the auto step)

Ashwin Raghunandan I agree with many others. The reason for this is that it Time/Date stamps the submission and we can track results.
In healthcare, if I ask you to submit your score at each appointment as "On a scale of 0-10, what is your pain score today?" Unfortunately the current methods overwrite the results. Attaching a PDF would allow me to look at previous days.
To enhance this functionality, saving that result in a Google Sheet or similar would also enhance utility. I could send my client a graph of progress, or internally monitor improvement. Very exciting options!
Regarding format- both would be great. But a link is a great way to start. And certainly attaching/storing this as a document on the contact record makes it useful

Ashwin Raghunandan Hi Ashwin In addition to all the other options, I would like a copy to be stored in the contact record, and someplace where we can build automations that will allow for us to do different things with the PDF to include sending it outside the GHL environment.
Additionally there may be aneed to store multiple PDFs separately, and still have access for automations, etc. So it would be nice to have the pfd uploads also be stored to file folders inside the media storage as long as the media storage. If this were the case, it would be nice to somehow automatically get the irl of the pdf in media storage so that it could be stored in a custom field for use in automations.

is this what you're looking to do as for a document Upload? yes you acn do it easily with firnd and add object field
im guessing you have everything on paper and wsnt ti upoad them
if you have anything in a spreadsheet it gets even easier once you setuo your forms to match that spreadsheet

Prasoon Dadhich When? This feature request dates back to Aug 29th 2022, and was marked as "planned" just shy of 3 years later. It's now 4 months past it being "planned." Can we get any kind of estimate as to when this long requested feature will actually be rolled out and usable?

I am having a serious trouble because i dont find a way to get a PDF attached with all information of the form submitted. This is something possible in wix, where you can fill a wix forms and automate to send an email with the information of the form in the email, but also as a PDF attached, including pictures, instead of the limitation with GHL forms that just send a link to the picture. This is urgent needed. Pictures are visible inside the PDF. Please, we need this function so urgent . Almost 600 persons are requesting this function.
