Add Surveys as a WordPress Plugin shortcode, just like Forms

The LeadConnector WordPress plugin creates a shortcode to be able to put a FORM anywhere on the site. Example:
[lc_form id='abc123' title='Contact Form']
I want the same but for a SURVEY.
